A fall is the moment most families start shopping for a medical alert system, and the scenario everyone worries about is the one where the person can’t reach the button. Automatic fall detection is designed for exactly that: a sensor in the pendant or watch recognizes a hard fall followed by little movement and calls the monitoring center for you.
Almost every major company now offers it, usually as a monthly add-on. We compared what it actually costs, which devices support it, and the limits every buyer should know about.
Our quick picks
- Best overall: LifeFone. Fall detection on every device for just $5/month, with no equipment fees.
- Best for at-home use: Bay Alarm Medical. In-home system with fall detection for $44.95/month on cellular.
- Lowest total monthly cost: Lively Mobile2. $34.98/month with fall detection, plus the device upfront.
- Most device choices: Medical Guardian. Fall detection on home units, mobile pendants and a smartwatch.
- Best for big homes: MobileHelp. Up to 1,400 feet of range from the base station.
- Fall detection with no monthly fee: LogicMark Guardian Alert 911 Plus. A one-time purchase that calls 911 directly.

What to know before you buy fall detection
- It doesn’t catch every fall. Every company we reviewed says so. Slow slides out of a chair or a gentle fall onto a bed may not trigger it. The person should still press the button whenever they can.
- It has to be worn correctly. Most pendants work best worn around the neck. Lively, for example, says its fall detection only works when the Mobile2 is worn on the included lanyard, not clipped to a belt or carried in a bag.
- Expect some false alarms. Dropping the pendant or sitting down hard can trigger a call. The monitoring center will talk to the person first. Ask whether the company charges for false alarms before you buy.
- Batteries drain faster. Bay Alarm Medical, for example, says its fall detection buttons last about 2 years versus up to 5 years for a standard button, and mobile devices with fall detection need charging more often.
- It’s not a substitute for prevention. Talk with a doctor about fall risk. Grab bars, good lighting and a review of medications can reduce falls in the first place.
LifeFone: Best overall
LifeFone charges the least for fall detection of any company we compared: $5 per month on every device, including its at-home systems, mobile pendants and smartwatch. With no equipment or activation fees, it also has the lowest first-year cost for a monitored mobile device with fall detection that didn’t require buying hardware.
Pricing
- VIP Active mobile pendant: $37.95/month (annual plan) + $5 fall detection = $42.95/month
- At-Home Landline: from $24.95/month + $5; At-Home Cellular: from $30.95/month + $5
- Safe Watch Active smartwatch: from $43.95/month + $5
- No equipment or activation fees
Why we like it
- Half the fall detection price of most competitors

- Fall detection is available on its smartwatch, which is rarer than on pendants
Who it’s not for
- People in areas with weak AT&T or Verizon coverage, if they need the mobile devices
- Anyone who wants to avoid return shipping costs: if you send equipment back in the trial, you pay to ship it
Bay Alarm Medical: Best for at-home use
For someone who spends most of the day at home, Bay Alarm Medical’s SOS Home is a strong pick. The cellular version with fall detection costs $44.95 per month, there’s no device fee on the base system, and the waterproof button works up to 1,000 feet from the base, including in the shower.

- Elite mobile pendant: from $37.95/month + $10 (a phone-only promotion at the time of writing offered the Elite with free fall detection for $35/month)
- Free activation and no upfront equipment cost
Why we like it
- Longest in-home range in this guide
- Nothing to pay upfront for equipment

How we chose
We compared the medical alert companies families search for most on fall detection price, which devices support it, total monthly cost, upfront fees, trial periods and cancellation terms. Prices were checked on each company’s own website on November 1, 2026. Where a company didn’t list a detail online, we used recently published plan details and say so.
Our guides are based on research, not hands-on testing, and we can’t verify how accurately any device detects falls. We may earn a commission from some links on this page, but it never changes our rankings. See how we make money.

Frequently asked questions
How much does fall detection cost?
Most companies charge $5–$10 per month on top of the base plan. LifeFone was the cheapest we found at $5/month. Devices with no monthly fee, like the LogicMark Guardian Alert 911 Plus, include it in the purchase price.
How accurate is fall detection?
No device detects every fall, and companies say so in their terms. It works best for hard, sudden falls when the device is worn as directed. Treat it as a backup to pressing the button.
Does fall detection work in the shower?
Many fall detection pendants are waterproof and made to be worn in the shower, where falls are common. Check the specific device’s water rating before you buy.
Is fall detection worth it?
For someone who lives alone, has fallen before, or has a condition that affects balance or consciousness, it’s usually worth the extra few dollars a month.
